The State of Illinois college Immunization Code (77 ILL.ADM.CODE 9=694) and Trinity Christian College require that specific health/immunization records be on file in Student Life PRIOR to the start of the semester.  Students enrolled in undergraduate or adult studies (with 9 or more credit hours) or graduate programs (with 6 or more credit hours) are required to complete the Immunization Records Form and the highly recommended COVID-19 Vaccination Record.  Full instructions for submission of Immunization Records can be found under the ADMITTED STUDENTS section.

Once you submit your records, they are kept on file in Student Life. Due to the high volume of records being submitted, it may take an extended amount of time for your records to be processed. Trinity Christian College does not provide confirmation of records received.

Certain students are exempt from this requirement:

Types of Exemptions

• Age: If the student was born before January 1, 1957, an age exemption applies for measles (rubeola), mumps, and rubella (German Measles) only.
• Medical: In place of immunization records, you may claim an exemption by submitting documentation completed by you AND a licensed medical provider on the Medical Immunization Exempt Form.
  • In case of pregnancy or suspected pregnancy: Your clinician should complete the form indicating that the student is pregnant or suspected pregnant and an approximate due date.
  • In case of medical contradictions: Your clinician should complete the form indicating the specific contradictory vaccine(s) and duration of the medical condition.
• Religious: You may claim an exemption for religious reasons by completing the Religious Immunization Exempt Form.    A signed, and dated statement by the student (and parent/guardian if the student is a minor) describing her/his objection to immunization on the grounds that they conflict with the tenet and practices of a church or religious organization will suffice in place of immunization records.
• Online Graduate Students:  Graduate Students who are enrolled in an online-only program are exempt from the immunization records submission requirement, and therefore do not need to submit immunization records. You only qualify for this exemption if ALL COURSEWORK is completed online.

5. What do I do if I do not meet Illinois Immunization requirements?
   Under the Affordable Care Act, health insurance plans are required to cover recommended preventive services without charging a deductible, copay, or coinsurance when service is provided by an “in-network” provider. Adult immunizations such as MMR vaccines, Tetanus, Diphtheria, acellular Pertussis (Tdap), and meningococcal vaccines are included under recommended preventative services. For the most affordable immunization administration option, obtain a list of “in-network” providers from your health insurance plan and schedule an appointment. Retail clinics like Walgreens, CVS, and Target as well as your local public health department, may be another option for obtaining the immunization you need. If using this option, please note that receipts of purchase for vaccination do not fulfill the requirement. Documentation must show that vaccine was administered along with the healthcare provider’s signature.
6. What happens if I do not submit my Immunization Records prior to the first day of classes? Students who have not submitted all required immunization records will receive an email to their Trinity account reminding them of the required actions steps needed to submit their immunization records.  Failure to provide the required documents within the specified time frame will result in an Immunization hold (IM hold) and a $100 fee being applied to your account.  It is in your best interest to submit the required immunization records to the Student Life office on time and as promptly as possible to prevent delays in registration and to assist us to meet the state health requirements.

1. Do I need a physical to go to Trinity?

No physical exam is required for general admission to Trinity Christian College, however, all students planning to participate in intercollegiate athletics at Trinity must have a yearly physical on file with our Athletics Department prior to athletic participation.

2. Do I need to show proof of health insurance?

No, we do not require students to provide proof unless they are a student-athlete or international student.

3. Are there health resources available if I am not feeling well?

Trinity is in a prime location for medical care, with several medical providers within 10 minutes of campus.  Trinity does not specifically endorse any health providers.  The list below is provided for informational purposes and because of their proximity to campus.

• We enjoy a partnership with Health Center (sxu.edu) in which students may utilize Saint Xavier’s health center. If insurance coverage is an issue, SXU offers a fee-for-service option.
• Ingalls Care Center in Crestwood is just a couple of miles down College Drive.
• Palos Community Hospital, a top hospital, is just 2.5 miles from campus.
• CVS Minute Clinic: Inside their pharmacy at 6748 WEST 111TH STREET WORTH, IL 60482. This location is just 2.4 miles from campus and can handle routine concerns.

• A licensed clinician will need to complete a portion of the immunization form. Please allow yourself adequate time to complete this process.
• Adult Undergraduate and Graduate Students can bring the completed immunization form with you to orientation.

1. Why must students provide this information?
   We must report to the state that all students are in compliance with its requirements. In the unlikely event of an outbreak on campus, the Health Department uses our records to determine who is/is not permitted on campus.

     Contact your healthcare provider to request an antibody titer (blood test) for measles, mumps, and rubella. This test will confirm whether you have immunity to these viral diseases. Please note that all antibody titer results reported in the “equivocal” are not acceptable as proof of immunity and will require further written clarification by your healthcare provider.  A copy of the lab report confirming immunity and all clarifications must be submitted to Student Life by fax or through the student portal.
   • Tetanus, Diphtheria and Pertussis
     If you have been in the Illinois school system since 1989 it is likely that you have already had most if not all required immunizations. The only exception is the time-sensitive Tetanus, Diphtheria (Td) or Tetanus, Diphtheria, acellular Pertussis vaccine (Tdap) which is not acceptable if administered 10 or more years ago.
   • Re-vaccination/booster: Revaccination will be necessary if professional documentation of up-to-date immunizations are not available. The State of Illinois requires documentation of the month, day and year that the vaccine was administered along with a signature of the health care provider that administered the vaccine.

We strongly encourage you to keep a copy of your submitted records.  However, if you are unable to locate your Immunization Form you may request a copy of your immunization records to be sent to your home from the Student Life Office by completing the Medical Release of Information Form (and faxing it to the Student Life Office at 708.293.8761.

Student Health Insurance for ALL Trinity Students (Traditional, Graduate, Adult)

All Trinity students are required to have health insurance. Please consult with your health insurance provider to be sure you have adequate coverage for your time at Trinity. Some plans only cover students in their home state. We advise that you secure a plan that covers your health care while you are physically at Trinity in the state of Illinois. Domestic students are not required to show proof of insurance, because individual coverage is required by law.

Student Health Insurance for International Students

Please note that all students are Trinity are required to carry health insurance, including international students. Trinity makes a specific plan available, but you are also able to find an international student plan on your own, provided that it meets specific requirements (this is not an option for student-athletes). Whichever policy you choose must include the repatriation of remains.

International students studying at Trinity have the following options for obtaining health insurance:

1. Student-Athletes:
   If you are a student-athlete playing for a Trinity team, we have a new policy: You must sign up for the ISP for Athletes Insurance you may pick either of the two options.
2. Students who are NOT part of a Trinity Athletic Team:
   Find a provider of your choice that has sufficient coverage for studying in the states. F-1, R-2, and J-1 visa holders who are not student-athletes may sign up for any insurance plan with the only necessary requirement being that the plan covers the return of mortal remains. Trinity encourages students to review plans carefully and select a plan that meets their needs in addition to the requirement of coverage for the return of mortal remains. ISP International Student Protection (the ISP Trailer Blazer policies meet the minimum requirements set by Trinity). Some other insurance providers students have used in the past include Compass student insurance and International student insurance (ISOA) but these have not been preapproved so you must check that the policy you select meets the minimum requirements stated above. It is possible to find reasonably priced options. If you choose to find your own provider, you must send the summary of benefits to International@trnty.edu for pre-approval before purchasing, to make sure that you have the coverage you need.Once you have a valid insurance policy, you must upload a copy of your insurance card and insurance policy coverage into your Student Portal.

Where and how can I register my vehicle?

Traditional Undergraduate: Registration can be accessed on your student portal through trollweb.trnty.edu or portal.trnty.edu. Please be sure to double check that you have registered your vehicle for the correct school year term once you log in.

Adult and Graduate students:  Can access the Vehicle registration form online  or can fill it out at Orientation. Parking passes will also be distributed at Orientation.

These forms provide clarification for our expectations of you while receiving an education here at Trinity Christian College and provides us with critical information about you such as emergency contact information and the different consent forms that must be on file.
